Forney ISD will hold a weekly COVID vaccine clinic on Thursdays with varied hours. Please see the schedule below. Pfizer and Moderna vaccines will be offered at the Forney ISD Administration Boardroom located at 600 S. Bois D’Arc Street.
Dates and times
• December 2: 7 a.m. - 12 p.m.
• December 9: 7 a.m. - 12 p.m.
• December 16: 7 a.m. - 12 p.m.
You must register at the link provided at the end of this article to receive your first dose of Moderna, Pfizer or a Booster vaccine for all those eligible persons to receive the respective vaccine at the Forney ISD Thursday clinic, including the pediatric Pfizer vaccine for 5-11 year old children.
The adolescent and adult Pfizer vaccine is available for students 12 years of age and older. Students under the age of 18 must have a parent/guardian present. Parents will need to provide their photo ID and will need to sign a consent form for their student. Moderna vaccine is available to all 18 years of age and older. The second dose of the vaccines will be scheduled at this same location. Booster vaccines are available. You must bring your white CDC vaccine card for second and booster doses to be given.
